The SWI position provides case management and service coordination of Holocaust survivors, older adults, and clients with disabilities and their families.

 QUALIFICATIONS:

  • Bachelor’s degree in social work or related health or service-related field 
  • Current unrestricted CA Driver’s License and proof of auto insurance
  • Prior experience with client engagement, customer service, or social service interventions, and working with the LGBTQ community is desired.
  • Bilingual Russian preferred.
  • Demonstrating front-facing customer service skills, being culturally sensitive, and being able to work as part of a team and maintain professional boundaries.
  • Experience working with older adults, caregivers, and mentally and physically disabled clients is highly desired.
  • Excellent verbal and written communication skills and ability to utilize telehealth platforms.
  •  Must be computer literate in MS Office and have the ability to learn new computer applications.
  • Knowledge of community resources and the aging process
  • Knowledge of safety issues when working with at-risk clients
  • Ability to work collaboratively as part of an integrated team.
  • Ability to maintain professional boundaries.
  • Ability to utilize a strength-based, non-judgmental, and Person-Centered Approach
  • Flexibility and ability to work autonomously.
  • Must pass a criminal background check (Live Scan fingerprinting) before beginning employment.

 

 

 

RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

As assigned, under the supervision of the Director of the West Hollywood Comprehensive Service Center or her designate, the Service Coordinator will be responsible for:

  • Provision of information and referral, concrete services and advocacy, short-term case management based on a targeted case management assessment, resource coordination, and benefits/ forms/applications assistance for individuals and families residing in contracted buildings to promote health, psychological, and social well-being.
  • Work closely with the housing building managers from all four sites to coordinate services for interested residents.  Outreach and education to building managers and residents on types of programs and services available in the community.    
  • Monthly check-in and ongoing monitoring of service delivery and outcomes.  
  • Manage multiple tasks and prioritize effectively.
  • Provide excellent, timely, and responsive customer service.
  • Participate in weekly individual supervisory and team meetings.
  • Appropriate documentation in the agency’s database, tracking statistics, and promptly preparing and submitting quarterly reports.
  • Other related duties as assigned.
